Let your commercial and Medicare Advantage clients know that ConnectiCare can help see that they are getting the care they need. Our care management nurses, certified diabetes educators, pharmacists, social workers and health care navigators are available for members by phone and now in person at our Manchester ConnectiCare center.
Care managers can help members who have questions about or need help with:

Managing a condition or illness, like diabetes, asthma, COPD, or heart disease |
• |
Upcoming surgeries or procedures |
• |
Follow-up care after a stay in the hospital or a skilled nursing facility |
• |
Rides to doctor appointments |
• |
Filling or paying for prescriptions |
• |
Finding urgent care centers for non-emergencies |
• |
Quitting smoking |
• |
Finding local social and support services, including food banks and subsidized grocery programs, transportation services, support groups, prescription assistance programs, and senior centers and activities |
Members can call 1-800-390-3522 (TTY: 1-800-842-9710), 8 a.m. to 4 p.m., Monday to Friday, to ask for help. Outside of those hours, they can leave a message with a time to call them back.
Members can also make appointments to meet care managers at the ConnectiCare center in Manchester. Appointments are available from 9 a.m. to 4 p.m. on Tuesdays to visit with a nurse, on Thursdays to meet with a social worker, and on the second Wednesday of the month to visit a certified diabetes nurse educator. Members should call the center at 1-877-523-6837 (prompt 2) to make appointments.
