Working to address the opioid epidemic

ConnectiCare launched an advanced opioid management program in January to help members safely treat their medical conditions while limiting the potential for drug misuse. The goal is to improve communication among pharmacies, prescribing health care providers and members.

The program includes:

Clinical reviews and provider notifications for members exceeding a total morphine-equivalent dose of more than 200 mg per day.
A seven-day supply limit for the first fill of short-acting opioids.
Real-time alerts to dispensing pharmacists if there are concerns about a member’s clinical safety or medication use.
Helping members bring all prescriptions under one treating provider, if needed or applicable, for better care management.

This program applies to all ConnectiCare members. Members may receive letters from Express Scripts, our pharmacy benefit manager, about this program. Contact your sales representative for printed copies of our flyer describing the program.

ConnectiCare launches new program with Cityblock Health

ConnectiCare is partnering with Cityblock Health to bring a new care model to Connecticut—one that addresses the social determinants that can prevent members from getting the health care and support they need.

Together with Cityblock, ConnectiCare will identify members with unique and complex health and social needs, and provide those members with medical, behavioral health care, and social services tailored to their specific needs.

Cityblock’s care teams meet members wherever and whenever is most convenient for the members—at home, in the community, even by phone or video visit. Cityblock partners with local organizations to address barriers to care, such as child or elder care responsibilities, housing, nutrition and transportation.

Beginning in Waterbury and the surrounding area, we will offer a small panel of commercial and Medicare members the opportunity to participate in the new program. When ConnectiCare members elect to participate, Cityblock will reach out to the members’ doctors about working together to advance their patients’ health and well-being.

Housecall member newsletter on its way

ColdFlu

Your Medicare Advantage clients will soon receive the Winter 2019 issue of Housecall. This printed newsletter includes: tips to maximize pharmacy benefits, when to use urgent care, ideas for staying social, a checklist to guide physician conversations and more. The newsletter should arrive in homes in the next few weeks. Contact your sales representative for copies. This publication will not be sent to Passage Dual (HMO SNP) members.

D-SNP all-in-one kit available

A new all-in-one kit for individuals enrolling in the Passage Dual (HMO SNP) Medicare Advantage plan is now available. The kit includes an overview of key plan features, a summary of benefits and enrollment forms. The kit is available in English and Spanish (coming soon). Contact your sales representative or email brokerservices@connecticare.com for printed copies.
