1095 tax forms to be mailed to your clients

Your clients will receive their 2020 proof-of-coverage tax forms in the mail soon. The 1095 forms show what months the subscriber and their dependents were covered by the health plan.

Access Health CT has mailed 1095-A tax forms to individual members with coverage through the health insurance exchange. Exchange members can also view a copy in their secure account inbox.

ConnectiCare will mail 1095-B tax forms to SOLO members and members of fully-insured group plans by the end of February. These members will also be able to view their forms after signing in to the secure member website through connecticare.com.

If your clients changed health plans during 2020, they may receive multiple forms from the different plan. They should keep a copy of all 1095 forms for their tax records.

Note: ConnectiCare will mail 1095 forms for employers with Fixed Funding Solutions plans unless the employer has opted out. Other self-insured group plans will mail 1095 forms to their employees.

2021 ConnectiCare Benefits, Inc. plans provide lower cost plan options for small group employers

Provide your small-group clients with more cost-saving plan options. ConnectiCare Benefits, Inc. (CBI) plans are among the most competitively priced fully insured plans on the market. Consider including a CBI plan as you quote fully insured plans for new and renewing small group clients. You can select CBI plans under the ConnectiCare option in BenefitCentral or HealthConnect, or by using the small group quoting tool in the ConnectiCare broker secure website*. View the CBI product grid for specific benefit details.

There are important distinctions to note when quoting CBI plans. These plans use the same network as plans sold through Access Health CT. This ConnectiCare proprietary network includes Connecticut and bordering part of Massachusetts and Rhode Island. The EmblemHealth Prime network in New York and the First Health® national network are not part of the CBI plan network.

Clients can go to Find a doctor to see participating providers. Here is a quick reference as to what to choose when using Find a doctor.

Choice Passage

Members who choose a CBI plan will follow the small group 4-tier employer sponsored drug formulary.

New or renewing groups who choose a CBI plan must complete the CBI employer application. Employees who select a CBI plan must complete the CBI employee enrollment form. Forms should be returned to cciadmin@cbia.com.

Remember, you must be appointed to sell CBI plans with ConnectiCare. If you have yet to become appointed, please send the completed producer agreement to CCIBrokerCommission@connecticare.com or via fax to 1-860-678-5224.

Visit CBIA’s website for more on fully insured small group plans.

*CBI plans are also sold through SHOP distributed by Access Health CT.
